HOW CAN A CLIENT BE ADMITTED TO OUR SERVICES?

A client may be referred to us by:

• A doctor, family or friend
• A social worker or discharge planner of the facility or hospital where the client is
• The client can directly contact us

Our nurse, the client, and his/her physician will work together to ensure the plan of care provides the best individualized treatment for the client.

HOW CAN A CLIENT QUALIFY?

The client may call us directly to discuss any health needs with one of our experienced nurses. Our nurses will assess if the client qualifies for home health care over the phone or by personally meeting with the client. With the consent of the primary care physician, the client may be admitted for services with our agency. In the rare event the client does not qualify for home health services, we will make every effort to refer him/her to the appropriate community resource that will be able to meet his/her needs. The cost of all our services may be covered by:

• MEDICARE
• Private Insurance / PPO
• Private Pay
