Title: Licensed Physical Therapist Assistant


POSITION SUMMARY
Responsible for the provision of assigned physical therapy procedures to patients in accordance with an established physician plan of care and under the coordination and supervision of a licensed physical therapist.


POSITION QUALIFICATIONS
1. Graduate of Associate Degree program for Physical Therapy Assistant approved by the American Physical Therapy Association with current licensure in the state(s) of practice.
2. One (1) year of experience as a licensed physical therapy assistant, preferably in an acute care setting.
3. Good written and verbal communication skills.
4. Valid driver's license in the state of Texas, available and dependable transportation and proof of current automobile liability insurance.
5. Ability to meet minimum agency hiring standards which include evidence of current cardiopulmonary resuscitation (CPR) certificate and willingness to perform such procedures in the home setting.
6. Essential physical, mental, and / or environmental working conditions:
a. Must be able to meet the physical demands which including standing, walking, bending, lifting, and transferring.
b. Must be able to utilize good judgement, demonstrate patience, and maintain a professional demeanor.
c. Must be able to work in a busy and stressful environment.


ESSENTIAL JOB FUNCTIONS

 
1. Provide physical therapy assigned services to patients in the home setting, accordance with a physician plan of care and under the supervision of a licensed physical therapist.
a. Assists in client treatment according to physician orders and assignments made by the licensed therapist.
b. Observes, documents and reports client reaction and response to therapy to the supervising therapist and / or nursing supervisor.
c. Completes all scheduled visits / treatments as assigned.
d. Collaborates with the therapist in developing treatment plans and short and long term goals.
e. Reassesses treatment plan with assistance from the licensed physical therapist relative to necessary modifications and adaptations based on client status.
f. Assists in the determination of available community resources for appropriate referral according to client need.
g. Implements home program specific to individual clients needs and educates the client / family accordingly.
h. Collaborate with the physician and home health nurses to discuss client care therapeutic modifications and discharge planning needs as assigned by the licensed physical therapist.
i.  Participates in regular client care conferences under the direction and at the request of the licensed physical therapist.
 
2. Completes accurate and timely documentation for the patient's medical record and billing purposes.
a. Records observations and actions on regular progress notes in accordance with established agency guidelines.
b. Documents teaching plan and client response to therapy.
c. Ensures that all visits, assessments dates, treatment activities, and evaluations are documented in the clinical record.
d. Reviews client progress at least monthly and adjusts care plan as instructed by the licensed physical therapist.
e. Attends monthly client care conferences as requested by licensed physical therapist and / or supervising RN.
 
3. Maintains current knowledge of professional skills and seeks continuing opportunities for skill enhancement.
a. Maintains current knowledge of safety measures, infection control guidelines and policies and procedures governing delivery of service to the client.
b. Maintains certification with the American Physical Tehrapy Association as a certified physical therapist assistant.
c. Keeps current on clinical developments and regulations affecting the provision of physical therapy services.
d. Practices infection control and follows all policies regarding universal precautions.
e. Adapts to altered work patterns whenever needed.
f. Demonstrates a consistently well organized approach to designated schedule.
