Title: Occupational Therapist


POSITION SUMMARY
To ensure quality and safe delivery of home health care services and assist in the provisions of home health care services that reflect the agency philosophy and standards of home health and nursing care of assigned clients. To provide quality and safe delivery of home health occupational therapy services.


POSITION QUALIFICATIONS
1. A registered Occupational Therapist by the American Occupational Therapy Association or a graduate of a program in occupational therapy approved by the Council on Medical Education of American Medical Association in collaboration with the American Occupational Therapy Association and engaged in the required supervised clinical experience period prerequisite to registration by the American Occupational Therapy Association.
2. Current licensed occupational therapist with the State of Texas Board of Occupational Therapists required.
3. Current certification in cardiopulmonary resuscitation (CPR), yearly TB or Chest x-ray.
4. One year minimum experience preferred, in a hospital or related health care field.
5. Valid driver's license in the state of Texas, available and dependable transportation and proof of current automobile liability insurance.
6. Essential physical, mental, and / or environmental working conditions:
a. Must be able to meet the physical demands which including standing, walking, bending, lifting, and transferring.
b. Must be able to utilize good judgement, demonstrate patience, and maintain a professional demeanor.
c. Must be able to work in a busy and stressful environment.
 


ESSENTIAL JOB FUNCTIONS

1. Ensures quality and safe delivery of homea health care services.
a. Participates in development and implementation of client plans of care per home health care agency policy and procedure.
b. Participates in client case conferences according to home health care agency policy and procedures as appropriate.
c. The provider of occupational therapy care reflects the client plan of care.
d. Information regarding client plan of care is submitted to the Registered Nurse in a timely manner.
 
2. Implements current rehabilitative occupational therapy practices.
a. Client's plan of care is discussed with the Registered Nurse on a regular basis.
b. Rehabilitative interventions are implemented for client plan of care.
c. Client clinical records are documented per home health care agency policy and procedure.
d. Client assignments and reports are received from the RN or the Intake Coordinator.
e. Infection control and safety policies and procedures are implemented per home health care agency policy and procedure.
f. Identified client needs are communicated to the RN in a timely manner.
g. Clinical care is implemented per home health care agency policies and procedures.
h. Client's overall appearance reflects good personal hygiene.
i. Client environment is neat and orderly.
j. Procedures and treatments are implemented and documented appropriately.
k. Observation of client conditions are accurately reported to the Director of Nursing or Nursing Supervisor.
l. Clients are assisted to meet socialization needs.
m. Assignments are organized and completed on time.
n. Client requests are responded to promptly.
o. The RN is notified immediately upon any significant change in the client condition.
p. Reports on the client progress are given to the RN on a weekly basis or as indicated.
 
3. Fulfills additional performance responsibilities as assigned and as necessary.
a. Additional performance responsibilities are fulfilled accurately and in a timely manner.
 
4. Promotes effective written/verbal communication.
a. Skill in conflict resolution is demonstrated.
b. Promotes a positive work environment for the agency staff members.
c. Participation in monthly meetings is demonstrated and documented.
d. Changes in agency operations and policies and procedures is communicated to home health care agency staff members in a timely manner.
e. Consistent and concise lines of authority and responsibility are maintained.
f. Cooperation with agency staff members and the Director of Nursing pursuant to home health care agency philosophy and objectives is demonstrated.
g. Documentation of data regarding interpretation of agency philosophy, objective and long range plan is demonstrated as deemed necessary.
h. Reporting to the Director of Nursing is initiated as issues and concerns require.
 
5. Accurate information is given to the client, and the client's family regarding costs for home health care services.
a. Consultation with the Director of Nursing regarding financial matters is demonstrated.
 
6. Provides informaiton regarding equipment and supply needs of the home health care agency.
a. Equipment and supply needs are reported to the Director of Nursing in a timely manner.
b. Recommendations for capital expenditures are submitted in a timely manner to the DON.
c. Participation in the development of cost control policies and procedures is demonstrated.
d. Corrective action plans to deal with cost control problems are demonstrated.
e. Participation in the implementation of the agency budget to ensure operation within approved limits is demonstrated.
f. Cost reports are monitored and reviewed with the DON as necessary.
 
7. Utilizes equipment and supplies effectively and efficiently.
a. Utilization of equipment and supplies in a cost effective manner is demonstrated.
b. Information is given to clients and family members regarding the effective and efficient use of equipment and supplies.
c. Information regarding malfunctioning equipment is given to appropriate individuals.
d. Reimbursement information of equipment and supplies is given to the client and family members regarding the needs of the home health care agency.
 
8. Complies with home healt care agency policies and procedures.
a. Job performance by occupational therapists is demonstrated to be in compliance with policy and procedures.
b. Participation in developing administrative and clinical policies and procedures is demonstrated as appropriate.
c. Participation in implementing systems for evaluating home health care agency delivery of services is demonstrated.
d. Contract personnel services are monitored for compliance with agency policy and procedures.
e. Participation in developing and implementing emergency policies and procedures is demonstrated.
f. Confidentiality of client information is demonstrated per agency policy and procedure.
g. Infection control and safety measures are implemented per agency policy and procedure.
h. Current license of occupational therapists are maintained and documented.
 
9. Promotes education for agency staff members.
a. Participation in needs assessment of agency staff members is demonstrated.
b. Participation in development and implementation of educational programs to address staff members needs is demonstrated.
c. Educational programs are documented in the occupational therapists personnel files.
 
10. Promotes education for client/client family members and community needs are demonstrated.
a. Identification in educational programs to address identified client/client family and community education needs is demonstrated.
b. Participation in educational programs to address client/client family and community education needs is demonstrated.
 
11. Participates in personal professional growth and development.
a. Professional career objectives are developed and implemented.
b. Current knowledge regarding professional career development in the delivery of health care services is demonstrated.
c. Review of current literature regarding health care is demonstrated.
