Title: Educational Community Health Nursing Coordinator
Reports to: ADMINISTRATOR


POSITION SUMMARY
Manages and leads an educational and outreach program with focus on disseminating information impacting on the health and well being of our patients and the general public. Represents the health care agency to the Houston medical community at-large in outreach liaison and consultation programs. Receives referrals, coordinates and assigns all necessary disciplines, and maintains an accurate census.


POSITION QUALIFICATIONS
1. The candidate must have a baccalaureate degree in nursing.
2. Five (5) year minimum experience as an RN in acute care setting with exposure to geriatric and medical/surgical client care.
3. Strong editorial, written, verbal and interpersonal skills required to deal with customers, clients, physicians, co-workers and with business contacts.
4. Ability to meet minimum agency hiring standards which include evidence of current cardiopulmonary resuscitation (CPR) certificate and willingness to perform such procedures in the home setting.
5. Must be able to meet physical demands which include standing, walking, bending, lifting, and transferring.
6. Must be able to utilize good judgment, demonstrate patience, and maintain a professional demeanor.
7. Must be able to work in a busy and stressful environment.


ESSENTIAL JOB FUNCTIONS
1. Prepares, delivers, and/or coordinates staff education programs and staff meetings in areas requiring review and development.
2. Develops and maintains standards for all outreach efforts and customer services.
3. Ensures compliance with local, state and federal rules and regulations through constant dialogue regulatory officials and the medical community at-large.
4. Lectures and/or participates in health fairs and in-service training.
5. Provides information regarding agency services and hospital case managing to the general public, physicians, prospective clients and other staff, as appropriate.
6. Maintains current resource information regarding available community services to meet actual and/or potential client needs and when appropriate, coordinates referrals and/or services with these resources.
7. Verbally interacts with customers, clients and employees to promote effective customer relations and a positive image that contribute to agency success.
8. Responds rapidly and efficiently during any emergency situation to meet client or employee needs.
9. Performs as member of the Continuous Quality Improvement Committee; complies with all established policies and performs member responsibilities as assigned.
10. Accepts new assignments and tasks as necessary to ensure effective operation of the agency.
